Eligibility declaration by a disabled person

Shop4beds are pleased offer a VAT rebate of 17.5% on the purchase of any of our eligable products to those customers who are chronically sick, handicapped or disabled, this applies to illnesses that are deemed long term. Simply print the form or request a pre printed version by post, complete and return.

We will refund the VAT back to you via the same method you initially paid for your goods, this will be done within 7 days of receiving the completed form and order.

Customer Eligibility:

• Registered disabled
• Chronically sick (physical or mental impairment with a long term adverse effect on everyday activities or has been deemed chronic by a medical professional)
• Diabetic
• Terminally ill
• It does not include a frail elderly person who is otherwise able-bodied or any person who is only temporarily disabled or incapacitated, such as with a broken limb.

Please note there are penalties for making false declarations
